Information for our patients
Before the day of procedure, one of our nursing team will call you. We’ll make sure you feel ready for your operation, and have everything you need for your stay with us. This is your opportunity to ask questions about your procedure, and discuss any concerns you may have.

You will be contacted 1-2 days prior to your procedure to confirm your admission time, fasting details and will let you know of any out of pocket expenses associated with your procedure.
Pre-admission
Pre-admission is an important part of your hospital care. To ensure we can confirm your admission, financial and other arrangements, we ask that you:

Fill in the online admission form, or complete the admission form that you would have received from your doctor (download copy of patient information form), as soon as possible and sent it back to the hospital 7 days before. If it is less than 48 hours prior to your admission date please either fax the admission form to (03) 91159888 or email to admin@doncasterhospital.com.au.

Please have the following ready when you fill in your admission form:
  • Personal/Next of Kin details
  • Medicare Card
  • Funding details (e.g. DVA, Private Health insurance, WorkCover or self-funding)
  • Benefit details (e.g. pharmacy benefit card or pension card)
  • Item numbers if these were quoted by doctors rooms. Information your doctor supplied to you re implantable medical devices (e.g. prosthetic and disposables) – if applicable.
If you have any concerns please telephone (03) 9115 9888 between 8:00am to 8:00pm or send an email to admin@doncasterhospital.com.au.

If you have private health cover, please contact your health fund prior to admission to check for any excess or waiting periods. We know that health and billing charges can be difficult to understand and we are happy to assist in any way we can, however we also advise that you seek clarification from your doctor and health fund.
